Multidisciplinary Assessment Team

The Children’s Multidisciplinary Team or CMAT is the team for individuals 21 and older who still have complex medical needs.

The CMAT will assess the need for long-term services and its primary purpose is to evaluate medical and psychosocial needs to make a medically necessary determination of Medicaid.

The long-term medical care funded by Medicaid CMAT certification are:

  • Medical Foster Care (MFC),
  • Nursing Facility (NF), and
  • Model Waiver (MW).  

CMAT may staff a person with medically fragile or medically complex with a nurse or a health supervisor. Medical documentation must support long-term care.
